Does TikTok use your SIM card to determine location? Yes. TikTok says that when Location Services are unavailable or turned off, it can continue estimating approximate location using device and network information such as SIM card region, IP address and device system settings. TikTok's privacy policy also lists mobile carrier information among technical data it collects.
What TikTok officially says about SIM-card location
TikTok explicitly names SIM card region as a signal it may use when estimating approximate location.
TikTok also names the device's IP address. Changing only your SIM does not change the public IP of a Wi-Fi connection.
System settings are another signal. Where available and permitted, device Location Services can provide approximate or precise location information.
Does the SIM card determine your TikTok account region?

How does a SIM card identify a country?
Mobile networks use MCC (Mobile Country Code) and MNC (Mobile Network Code) identifiers. The MCC identifies the country or geographic allocation and the MNC identifies a network within that MCC. Together they form a PLMN network identifier.

Do you need a SIM card for TikTok?
No. You do not need a SIM card simply to use TikTok on Wi-Fi, and a country-specific SIM is not required to reach viewers in another country. TikTok can still estimate approximate location from other information such as your IP address and device settings.
A country-specific SIM is useful when your goal is to provide a consistent SIM-region/carrier signal on a real mobile device—for example, when testing how a device behaves with a particular regional setup.
Will changing your SIM card change your TikTok region?

A SIM and a VPN change different things. A SIM card changes the SIM/carrier region signal; a VPN changes the IP address seen by internet services.
